You'll have to be more specific. Is the menu not appearing in-game? Are karma points not increasing/decreasing properly? Is there some script error being thrown somewhere?

Removing the "Down with the Sickness" moodlet will effectively cure the Sim. This can be done by casting a Sunlight Charm, using the Moodlet Manager, activating the Good Times power, or using cheats.
